Cardiovascular and respiratory diseases are the most prevalent conditions that compromise dental patients. This seminar addresses basic pathophysiological principles that provide a foundation for preoperative assessment. Glass and his co-workers discovered that microorganisms that were cultured from patients’ infections were also found on their toothbrushes. A host of both in vitro and in vivo studies followed which showed that bacteria and yeasts not only contaminated the toothbrush, but also could proliferate there, especially in the moist, dark environment of the bathroom. The importance of these studies will be examined in the light of not only local dental disease, but also in relation to its effect on the heart and vascular system. The toothbrush will also be considered in its possible role as a bioterrorism instrument by considering the anthrax issue. Dr. Glass’ most recent studies have shown that dentures/appliances may not only harbor large numbers of pathogenic microorganisms, but may transmit disease-producing microbes to the oral mucosa and throughout the body. The discovery that white powder contained in several envelopes was Anthrax, and the subsequent deaths of several people who came into contact with it, brought the era of bioterrorism into the forefront. The news is replete with reports of nuclear proliferation and unusual flu-type epidemics. Each of these issues may cause concern for dental personnel, but even more critical is the important role that well trained dental personnel may play in response to a bioterrorism attack. Oral cancer continues to be a significant health problem in the United States. Approximately 30,000 new cases of oral cancer are diagnosed each year. Currently, one-third of those patients who receive a diagnosis of oral cancer will eventually die of their disease. The long-term survival of oral cancer patients is directly proportional to early detection, diagnosis, and treatment of the disease. Dental health care workers are uniquely positioned to detect the presence of oral cancer and its precursors, thus providing for significantly increased long-term survival. In addition, dental healthcare workers are intimately involved with the management of oral complications from both oral cancer and therapy, thus having a significant impact on reducing the morbidity of the disease and its treatment.
This course is designed to provide dentists, dental hygienists, and assistants with information on various aspects of oral cancer and related premalignant disease.
